From my professional experience as a User Experience Designer, I have designed these assessment pages that are simple and straightforward to test the factors that play the greatest part in hands-on testing. Tips
General Evaluation
For the general evaluation, stick to simple checks or percentages. The client wants a quick answer first before diving into details.
Table of actionable, specific recommendations or notes.
Although the list has 3 rows, it can have more or less depending on your findings. Delete what you do not need. If the principle has passed, and further comment is not needed, you can delete these tables altogether. Or, you can leave them for areas that are doing well and you want to encourage the client to keep doing them.
